Penerapan Telenursing Kontrak Modifikasi Perilaku Manajemen Hiperglikemia pada Pasien Diabetes Mellitus
Abstract
Program pengabdian masyarakat ini berfokus pada manajemen hiperglikemia pada pasien diabetes mellitus (DM) melalui pendekatan telenursing. Tujuan program ini adalah meningkatkan pengetahuan, kepatuhan, dan motivasi pasien dengan menerapkan kontrak modifikasi perilaku dalam layanan keperawatan jarak jauh. Program ini menggunakan pendekatan partisipatif, dengan kegiatan berupa edukasi digital, pemantauan berbasis WhatsApp, dan sesi tindak lanjut berkala. Selama empat bulan pelaksanaan, tercatat peningkatan pengetahuan pasien sebesar 85%, kepatuhan terhadap modifikasi perilaku mencapai 75%, dan penurunan komplikasi terkait hiperglikemia sebesar 20%. Pasien juga melaporkan motivasi yang lebih tinggi karena dukungan berkelanjutan dari tenaga kesehatan. Beberapa kendala muncul, seperti kekhawatiran pasien terkait privasi data, namun hal ini diatasi dengan komunikasi fleksibel melalui WhatsApp. Program ini menunjukkan potensi telenursing dalam memperluas akses layanan kesehatan dan menjadi model berkelanjutan untuk manajemen penyakit kronis di komunitas terpencil.
