Continuity of Care pada Ibu Hamil untuk Mencegah Perdarahan Post Partum di Puskesmas Kembang Kabupaten Jepara
Abstract
Perdarahan Post Partum adalah perdarahan yang terjadi pada masa nifas dengan jumlah perdarahan > dari 500 cc. Kejadian ini bisa terjadi karena ibu hamil yang mengalami anemia, yang dapat menyebabkan gangguan his baik primer maupun sekunder. Paritas atau jumlah yang dilahirkan baik itu lahir hidup maupun lahir mati. Seorang ibu yang sering melahirkan mempunyai resiko lebih besar mengalami anemia kalau tidak memperhatikan nutrisi selama kehamilannya, karena selama hamil zat-zat gizi tentunya akan terbagi dengan ibu dan bayinya. Continuity of Care (CoC) dilakukan bagi ibu hamil untuk memutus kematian karena 3 terlambat (terlambat memutuskan pencarian layanan kesehatan, terlambat mencapai tempat layanan kesehatan, dan terlambat mendapatkan pelayanan kesehatan). Tujuan kegiatan Pengabdian Kepada Masyarakat ini yaitu meningkatnya pengetahuan dan kesadaran masyrakat tentang pencegahan perdarahan post partum melalui optimalisasi Continuity Of Care pada ibu hamil dengan anemia Metode pelaksanaan Pengabdian Masyarakat CoC Pada Ibu Hamil Untuk Mencegah Perdarahan Post Partum” dilaksanakan pada tanggal 2 Oktober 2023 sampai 3 Desember 2023 di Puskesmas Kembang Kabupaten Jepara. Kegiatan pengabdian masyarakat ini dibagi dalam 4 sesi, yaitu sesi edukasi, sesi praktik, sesi pendampingan dan sesi monitoring serta evaluasi. Jumlah peserta yang mengikuti kegiatan pengabdian masyarakat ini berjumlah 20 orang ibu hamil dengan anemia. Kadar Hamoglobin (Hb) pada 20 orang peserta mengalami peningkatan menjadi normal dan sudah tidak mengalami anemia sehingga pada saat persalinan ibu juga tidak mengalami perdarahan post partum.
