Continuity of Care pada Ibu Hamil dengan Pre-Eklampsia di Puskesmas Keling II Kabupaten Jepara

Abstract

Menurut data dari World Health Organization (WHO) dalam Maternal and Reproductive Health sekitar 800 wanita meninggal setiap hari karena komplikasi kehamilan dan kelahiran. Penyebab utama kematian ibu (AKI) adalah perdarahan selama kehamilan atau persalinan, hipertensi atau preeklamsia, infeksi, dan penyebab tidak langsung, sebagian besar disebabkan oleh kontak langsung antara kondisi medis yang sudah ada sebelumnya dengan kehamilan. Hipertensi, proteinuria, dan edema atau pembengkakan pada tubuh adalah tanda-tanda penyakit kehamilan yang rumit yang dikenal sebagai preeklamsia, yang dapat muncul selama kehamilan atau hingga 48 jam setelah melahirkan, dan sebagian besar muncul pada trimester ketiga kehamilan. Untuk mengurangi jumlah penderita preeklampsia, penting untuk memberikan edukasi, pendampingan dan monitoring tentang komplikasi yang mungkin terjadi selama kehamilan pada rangkaian kegiatan COC(Continuity Of Care). Penyebab preeklampsia termasuk pola makan yang buruk, usia yang lebih muda/lebih tua, paritas, dan riwayat penyakit. Pada tahun 2023, terdapat 475 ibu hamil di Puskesmas Keling II, dan terdapat 92 residu dengan rincian: PEB 15, abortus 23, kelainan letak 3, serotinus 6, KPD 6, Retensio Plasenta 3, KPD 7, Hipertensi 1, Plasenta Previa 4, HIV 1, IUGR 3, Prematur 4, B20 1, IUFD 2, Gemeli 3, Diabetes Melitus 1, Gawat Janin 1, KPD 3, Post SC 5. Dalam upaya mencegah komplikasi dan mengenali kemandirian komunitas mitra (ibu dan kader terlatih), kegiatan pengabdian masyarakat ini bertujuan untuk meningkatkan pengetahuan dan kesadaran masyarakat tentang pengelolaan preeklampsia melalui optimalisasi Continuity Of Care bagi ibu hamil dengan preeklampsia. Hal ini akan membantu mempromosikan pengelolaan preeklampsia kepada ibu hamil lainnya (dukungan sebaya).
