Faktor-faktor Risiko pada Sepsis Neonatorum Awitan Dini
Abstract
Sepsis merupakan respon sistemik terhadap infeksi, terjadinya pelepasan mediator vasoaktif yang menyebabkan penekanan regulasi sistem saraf otonom sehingga menyebabkan vasodilatasi difus dan hipoperfusi yang dapat menyebabkan kegagalan multiorgan dan kemungkinan mengakibatkan kematian. Sepsis neonatal masih menjadi penyebab signifikan mortalitas dan morbiditas di Neonatal Intensive Care Unit (NICU) dan merupakan penyebab rawat inap yang lama meskipun telah menurun seiring dengan membaiknya perawatan neonatal. Pada Januari-Juni 2023 terdapat 120 bayi yang lahir dengan faktor risiko sepsis dan bayi didiagnosis sepsis awitan dini. Dari semua faktor risiko mayor dan minor, pada faktor risiko mayor KPD merupakan faktor risiko secara kuat yang mempengaruhi kejadian sepsis sebanyak 4,86 kali (RR = 4,86), demam intrapartum, dan korioamnionitis merupakan faktor risiko dari sepsis awitan dini dengan hubungan sedang (RR = 1,56, RR = 2,56). Pada faktor risiko minor skor APGAR rendah merupakan faktor risiko dengan hubungan kuat (RR = 4,32), usia kehamilan <37 minggu, dan infeksi saluran kemih merupakan faktor risiko dengan hubungan sedang (RR = 2,43 dan RR = 1,20). KPD >18 jam memiliki hubungan signifikan dengan sepsis (p=0,029).
